We offer same day appointments at all of our locations for urgent care needs. Our priority is to make every attempt possible in offering you a same-day appointment with your primary care provider at your home clinic. Please keep in mind that a same-day appointment may be offered to you at one of our other clinics with another provider to assist you with your urgent healthcare needs. Providers at each of our locations have access to your chart, and are happy to help you.

New Patients:

  1. Please call ahead to schedule an appointment for routine care. (Appointments can be scheduled up to 3 months in advance.)
  2. Save time and fill out your New Patient Registration Forms ahead of your appointment, Click HERE.

For your first appointment, please bring the following items:

  • Your insurance card (if applicable).
  • A list of your medications including over-the-counter medicine, vitamins, or herbal supplements. (Click here for an “Ask the Doctor” pre-visit checklist!)
  • Immunization records (for your child’s visit).
  • If applying for our sliding fee scale, please bring your current pay stub or tax information for income verification.

In the event of an emergency, please call 911.

Established Patients:

Established patients of any Full Circle Health clinic who sign up for MyChart can now schedule their follow-up appointments online!
