Pharmacy

Simply call any of our pharmacy locations with the names of your medications and your current pharmacy’s name and phone number. We’ll contact your old pharmacy directly and handle the transfer on your behalf. You don’t need to call your old pharmacy first.

When you call us, it helps to have:

• The name and dosage of each medication you want to transfer
• Your prescription number(s), if available (found on your pill bottle label)
• The name and phone number of your current pharmacy
• Your date of birth and insurance information (if applicable)

The more you can provide, the faster the process goes, but even if you only have your pill bottle handy, that will still help.

Most transfers are completed within 1-3 business days. Many happen sooner. We recommend starting the transfer before you run out of your current supply to avoid any gap in your medications.

Most maintenance medications can be transferred. Schedule II controlled substances, however, cannot be transferred by law – for those, your provider will need to send a new prescription directly to our pharmacy. Our staff can tell you which of your medications fall into this category and walk you through next steps.

No problem. If your prescription is out of refills, our pharmacy can contact your healthcare provider to request a refill authorization. Since your Full Circle Health provider and pharmacist are already on the same care team, this process tends to be faster than at outside pharmacies.

Sliding Fee Program:

Full Circle Health offers a sliding fee discount program to eligible persons based on the patient’s ability to pay. Ability to pay is determined by the household size and annual income relative to a discount schedule based on federal poverty income guidelines.

• Our pharmacists are directly communicating with your Full Circle Health provider and are following your treatment plan. (Our pharmacy staff can also speak multiple languages: Spanish, Farsi/Dari, Swahili, and Arabic)
• Mailed prescription options – No cost! (If insurance allows, restrictions apply)
• Automatic Prescription Refills (see below for more details)
• Short wait times: Only 5-10 minutes vs. 30-minutes at other pharmacies
• Prior Authorizations are FAST!
• Text Alerts or Voicemails when your prescription is ready.
• We also offer vaccines (call for availability)

When enrolled in autofill, you won’t have to call in to the pharmacy, our pharmacists automatically fill the prescription every month and request refills from your provider after the last fill.

   • Download the app to your smart phone (see options at the top of this page.)
2. Or, call your pharmacy or the phone number on your prescription label.

Please speak with your provider at each visit to make sure your prescriptions are up to date.  If you need a routine prescription refill, please contact your pharmacy and request a refill.   Your pharmacy will contact your health care provider’s office on your behalf – no need to contact the clinic directly.   Talk with your provider before your prescription runs out, and please allow 3 business days for refills. (Please note:  This is for routine prescription refills only, not life threatening circumstances.)
