Preventive Visits Notice

Preventive Visits Notice

We want you to know what to expect at your Preventive Health Visit (most commonly referred to as an “Annual Physical”, or for those under the age of 18, a Well Child Check (WCC). These visits help keep you healthy, find problems early, and plan for the future.
(This information may not apply if you have Medicare insurance.)

Adults

What’s Included in a Preventive Health Visit (Usually No Cost)

Most insurance plans cover many preventive services for no cost if you see a provider in your network. These visits focus on keeping you well and may include:

  • Talking About Your Health: We’ll ask about your past health, your daily habits, and any concerns you have.
  • Vital Signs & Physical Exam: We’ll check your blood pressure, heart rate, and temperature, and listen to your heart and lungs.
  • Lab Tests: Depending on your age and health, you may need lab tests. These are done by InterPath Laboratory, and you may get a separate bill.
  • Screenings: This could include cholesterol checks, diabetes testing, or referrals for cancer screenings like mammograms or colonoscopies.
  • Vaccines: We offer shots recommended by national guidelines (like flu shots).
  • Healthy Living Advice: You may get tips on eating well, exercising, quitting smoking, and more.
  • Medicine Refills: Your provider may refill medicines for ongoing health conditions.

These services are meant to prevent illness before it starts. They are not for treating health problems you already have.

What’s NOT Included

Your visit may cost more if we:

  • Treat a new problem (like back pain or a rash)
  • Order tests that aren’t preventive (like a CT scan or extra lab work)
  • Send you to a specialist
  • Change how we treat a long-term condition (like diabetes or high blood pressure)

If any of these happen, your insurance might charge you a co-pay or other fees.

Pediatrics

What’s included in a Well Child Check Visit:

  • Growth and Development Check: We’ll measure your child’s height, weight, and head size. We’ll also talk about how your child is learning, moving, and growing.
  • Physical Exam: A full check-up from head to toe, including eyes, ears, heart, lungs, belly, skin, and more.
  • Immunizations: Based on your child’s age and the CDC vaccine schedule, your child may receive shots to protect against illnesses like measles, polio, flu, and more.
  • Hearing, Vision, and Other Screenings: Some visits may include checks for hearing, vision, anemia, or lead exposure depending on your child’s age.
  • Health and Safety Tips: We’ll talk with you about feeding, sleep, screen time, car seat safety, school readiness, and anything else you’d like to discuss.

What’s Not Included:

Your visit may cost more if it is a:

  • Sick Visit: If your child is sick or has a new problem (like an ear infection or rash), we will need to schedule a separate visit or bill an additional charge.
  • Special Testing or Lab Work: Most lab work is not done at a Well Child Check unless your provider recommends it based on your child’s needs.
  • Referrals to Specialists: If your child needs to see a specialist, we’ll help you with the referral, but the visit will be billed separately.

Most Well Child Checks are covered at no cost to you by insurance when done with an in-network provider. If any extra services are needed, your insurance may charge you a co-pay or deductible.
