Isabel brings a strong background in Home Health, Hospice and Family Clinical care as well as medical surgical care to her role as a Transitions of Care Coordinator on the Care Management Team. She holds a bachelor’s degree in nursing, from Boise State University, as well as being a Board-Certified Nurse Coach through the ANCC. She also has an Associate’s degree in Business and Financial Management from Lambton College in Sarnia, Ontario, Canada.
Isabel is passionate about helping people navigate their own health and wellness, by exploring options to improve wellbeing through self-care and alternative therapies to reach optimal health outcomes. Outside of work, Isabel enjoys traveling with her dog and participates in the sustainability of our medical community, by helping them to navigate burnout and addiction. She is a member of the ISBN/DOPL Committee that monitors progression in recovery to promote sustainability of medical professionals, and encourages recovery from substance use. A big fishing and hiking fan, she’s always up for a new adventure.
