
Parker Brown, PharmD
Pharmacy Residency Program Director
Dr. Parker Brown graduated from Idaho State University College of Pharmacy 2013. He started his pharmacy experience in retail with Walmart for 4 years before getting the opportunity to move to a 340b pharmacy at what is now Full Circle Health. He came over part time, which also led him to work at some of the local psychiatric hospitals in town. For 3 years he was able to work with a multitude of patients at both places. In 2016 he was moved to a full time Clinical Pharmacist, a new position at Full Circle Health, and where he currently is working.
Current roles include instructing residents, DM pt, helping the current year pharmacy resident, and pushing clinical pharmacy across the organization. In his off time, you can find him at the lake or in the mountains wake surfing, or skiing.

Leili Izadi, PharmD
Head Pharmacist – Emerald / 340B Pharmacist
Dr. Leili Izadi grew up in Mashad, Iran, and graduated from School of Engineering with a degree in chemical engineering in 1998. Shortly after marrying her husband, Javad, she moved to America in 2005 to join him here in Idaho. Even though she was still learning the ins-and-outs of a new country and new culture, she began working in the Walmart pharmacy as a technician in 2006 and Started taking classes in Idaho State University she graduated from Idaho State University School of Pharmacy in 2012. Leili continued to work in the Walmart pharmacy as staff pharmacist until 2019 when she took up the position at Full Circle Health. Leili loves helping her patients and she knew immediately that Full Circle Health would be the perfect place to foster an environment to serve her community.

Alex Lipscomb, PharmD
Clinical HIV Pharmacist

Dr. Alex Lipscomb graduated from the University of Georgia College of Pharmacy in Athens, GA, in 2021 and the Full Circle Health PGY-1 Pharmacy Residency in 2022. After his residency he stayed on with Full Circle Health as the Clinical Pharmacist for the Wellness Center and Emerald Clinic. Alex takes pride in serving the underserved through Full Circle Health and providing guidance for new residents moving through the PGY1 program. When he isn’t working in the clinics, he likes to explore his city finding all the best spots for coffee, music, skating, and more. He enjoys hiking, photography, sneaker collecting, and pretending to know everything about vinyl records, but most of all he spends his time being the best role model he can be for his cat, Coco.

Jared Moore, PharmD
Director of Pharmacy Operations
Dr. Jared Moore was born and raised in Idaho Falls, Idaho. Dr. Moore graduated from Idaho State University College of Pharmacy in 2012. After graduation he worked at Shopko and Costco as a community pharmacist before joining Full Circle Health in 2016. He became the Director of Pharmacy Services in 2021.
His professional areas of interest include improving the patient experience through excellent customer service, building a diverse and talented pharmacy team, and finding new ways to generate revenue to fuel the mission of Full Circle Health. Outside of work he enjoys reading, biking, studying languages, travelling, stand-up comedy, and coaching his kid’s sports teams.

Dora Aguayo, PharmD
Head Pharmacist – Nampa
Dr. Dora Aguayo was born in Durango, Mexico. Her family moved to the United States when she was five years old. She grew up in Twin Falls, Idaho and began her undergraduate career at the College of Southern Idaho. She received her Bachelor’s degree in Biology with Health Care emphasis from Idaho State University and later graduated from the College of Pharmacy at Idaho State University. In her free time, she enjoys spending time outdoors with her husband and black lab.

Glenda Carr, PharmD
Clinical Associate Professor – ISU
Dr. Glenda Carr received her Doctorate of Pharmacy from the University of Montana in 2000. She completed her post graduate residency year 1 training at the VA Connecticut Health Care System and her post graduate residency year 2 specializing in ambulatory care at the William S Middleton VA Medical Center. She joined Idaho State University College of Pharmacy in 2002 and has been providing disease state management services to patients in various FQHC clinics. Dr. Carr joined the Full Circle Health team in 2021. Her professional interests include interprofessional education and curricular development. During her free time, you can find her watching any type of sporting event, preparing for an adventure, or sweeping a stone with her curling team.

Dave Hachey, PharmD
Clinical Professor – ISU
Dr. Dave Hachey graduated from the University of Rhode Island in 1998 where he earned his PharmD and completed a residency and fellowship in Family Medicine at Idaho State University in 2000. Currently, he is a professor at Idaho State University Kasiska College of Health Professions in the Department of Family Medicine where he manages a federally-funded program that provides care for patients living with HIV in eastern Idaho.

Roger Hefflinger, PharmD
Clinical Pharmacist

Roger Hefflinger, PharmD. Graduated from University of Nebraska Medical Center College of Pharmacy in 1986 then completed a clinical pharmacy residency also at the University of Nebraska. He joined Idaho State University College of Pharmacy in 1987 as a Boise Idaho (Treasure Valley) based faculty member and initially had a service commitment to Saint Alphonsus Regional Medical Center providing expertise in Nutritional Support and Intensive care unit medicine. In 1991 he switched service commitment joining the Family Medicine Residency of Idaho (FMRI) now Full Circle Health. Presently he has a joint affiliation with Full Circle Health and the College of Pharmacy. The Family Medicine Residency is a 40-resident physician, 50 faculty physician program with an emphasis on training the MD for rural practice.
Dr. Hefflinger has structured his educational experiences to provide pharmacy students and physicians with both in-patient and out-patient drug information, pharmaceutical care, and direct patient care management. His interest areas include Pain, Asthma, CAD, Infectious disease, heart failure, pharmacy cost containment strategies, and ambulatory medication management. Dr. Hefflinger contributes significantly to the regional continuing educational needs of physicians, pharmacists and mid level practitioners.

Matthew Kaiser, PharmD
Head Pharmacist – Meridian
Dr. Matthew (Matt) Kaiser first entered the pharmacy world at age 14 in a small independent pharmacy in North Dakota where he grew up. He went on to complete his undergraduate studies at North Dakota State University followed by his Doctorate of Pharmacy at the University of Montana. He went on to work as a staff pharmacist, pharmacy manager, and clinical services manager in 3 different states before landing in Idaho (ironically, his wife’s job in potatoes are what brought them here). Matt and his wife Katelynn enjoy traveling and hockey, especially traveling FOR hockey. Matt is the pharmacy supervisor at Full Circle Health’s Meridian Pharmacy.

Ammon Kunz, PharmD
Class of 2026
Dr. Ammon Kunz graduated from the College of Pharmacy at Oregon State University/Oregon Health & Science University in 2025. During his studies, he worked at an independent pharmacy and came to love interacting with his patients. This love led him to pursue a career where he can continue to meet one on one with patients, help them feel heard, and create personalized health plans for each of them.
Outside of work, he enjoys listening to audiobooks, going to the gym, and spending time with his wife and children. Together, they enjoy reading books, playing in the mud and sand, and going for walks to feed local chickens and ducks.
