
Samantha Portenier, MD
Program Director Caldwell
Samantha Portenier was born in Ontario, Oregon and grew up in northern Idaho. She went to undergraduate school on a rodeo scholarship and after graduating from Lewis Clark State College in Lewiston, Idaho did a year of graduate research at the University of Idaho before going to medical school at the University of Nevada, School of Medicine. In Nevada, she focused on primary care and rural medicine and returned to Idaho to do residency at the Caldwell Rural Training Track. After graduation in 2000, she joined the faculty of the family medicine residency and became the site director of the Caldwell Rural Training Track in 2004. She completed her specialized training as Program Director in 2013. When not at work, she is active with her family ranching, training performance horses and traveling to her three children’s rodeos where they participate.
Jocele Skinner, MD
Associate Program Director – Caldwell
Dr. Jocele Skinner was born in Nampa, Idaho and raised on a ranch in Eastern Oregon. Dr. Skinner graduated from The College of Idaho in Caldwell, Idaho, and then attended medical school in St. Louis, MO at Saint Louis University. The rural lifestyle and love of the high desert and sagebrush brought her back home to Idaho for residency. She attended the Full Circle Health Rural Track in Caldwell, and then stayed on to join the same medical group after graduation. She volunteers as the team physician in Homedale, the small farming town where she now resides. Dr. Skinner is married to George, and has two children, Catie and Jack. She has never been able to really give up ranch life, and her family has collected quite the menagerie of farm animals over the years. In her personal time, she enjoys knitting, cooking, reading, and gardening.

Richard Augustus, MD
Dr. Richard Augustus is a hospital medicine specialist in Caldwell, ID and has been practicing for 23 years. Dr. Augustus has been the Chief Medical Officer of West Valley since 2013. A graduated from University of Utah School of Medicine in 1996 he specializes in hospital and family medicine. He is passionate in training of the residents and the residency program.

Sulabha Chaganaboyana, MD, MS, DipABLM, FAAFP
Dr. Sulabha Chaganaboyana is a double board-certified physician in Family Medicine and Lifestyle Medicine who is passionate about helping patients improve their health through sustainable lifestyle changes. She enjoys partnering with patients, residents, and community members to make healthy living practical, accessible, and achievable.
Since joining family medicine residency education in Idaho, Dr. Chaganaboyana has worked to expand opportunities for Lifestyle Medicine training in residencies and patient care. She helped establish Idaho’s first Lifestyle Medicine Residency Curriculum and established Lifestyle Medicine teaching clinics and group visits to provide residents with hands-on experience in evidence-based lifestyle interventions.

Dr. Chaganaboyana has a strong interest in community engagement and enjoys sharing health education beyond the clinic. She has taught in schools, libraries, camps, and community organizations, focusing on topics such as nutrition, physical activity, and healthy habits. Through collaborative partnerships and local and national grant-supported initiatives, she has helped bring Lifestyle Medicine education to diverse populations throughout Idaho.
Her professional interests include culinary medicine, health coaching, physician well-being, and innovative approaches to medical education. She particularly enjoys leading cooking classes, movement sessions, and other interactive learning experiences that help translate medical knowledge into everyday practice.
Outside of medicine, Dr. Chaganaboyana enjoys applying the principles of Lifestyle Medicine in her own life, traveling, and exploring history and different cultures. She values lifelong learning, meaningful connections, and mentoring the next generation of family physicians.

Craig Cowling, MD
There are so many things that Craig enjoys, but chief among those is being a father to his 3 wonderful daughters. Teaching them how to live and explore their world is truly a joy! In Craig’s free time, he enjoys going to the mountains and keeping tabs on the Elk, Deer, Bighorn Sheep, and black bear populations around Boise. Long term goals include hospital and procedure based medicine, osteopathic and physical medicine, diabetes, weight loss, and pregnancy care.

Stacy Lamers, DO
HIV & Viral Hepatitis Fellowship Faculty
Stacy Lamers was born and grew up in Illinois. She received her undergraduate degree in biology from Drake University in Des Moines, IA then stayed in Des Moines to obtain her DO degree from Des Moines University. She completed a residency in family medicine at Genesys Regional Medical Center in Grand Blanc, MI. After residency, she moved to Idaho to work as a family medicine physician with St. Alphonsus Medical Group in Meridian, ID. Always interested in public health, she was fortunate to have had the opportunity to do the HIV and Infectious Hepatitis Fellowship at Full Circle Health and has remained to provide family medicine, HIV primary care, and OMT to patients in Caldwell and HIV primary care in Twin Falls. She enjoys spending time outside with her husband and two sons skiing and camping.

Jeff Edwards, MD
Dr. Edwards is originally from Alaska, a graduate of the University of Washington School of Medicine, and a residency at Full Circle Health. He then worked at St. Mary’s Hospital in Cottonwood, Idaho, for 12 years, providing full-spectrum family medicine. From 2005-2012, Dr. Edwards worked in rural Texas and completed a Master’s in Public Health degree at Johns Hopkins University.
In 2013, he joined Doctors Without Borders. He worked in various countries, including Kenya, Ethiopia, Liberia, Zimbabwe, Ukraine, and Afghanistan doing a combination of emergent global health response and public health-related research. Dr. Edwards is actively working with Doctors Without Borders and the World Health Organization (WHO) for approximately three months per year, doing monitoring/evaluation and public health research.
In 2015, Dr. Edwards joined the Department of International Health at Johns Hopkins University, School of Public Health, and completed his post-doctoral fellowship in 2017. He continues to work as an associate at John Hopkins University, teaching intermittently within the Department of International Health.
In 2016, he returned to Full Circle Health to work as faculty. In his spare time, Dr. Edwards enjoys his passions of backcountry skiing, biking, climbing, and traveling.

Michelle Miller-Burgess

Although I’ve called Idaho home since I was 5, my appreciation for the outdoors blossomed only recently, thanks to my wonderful husband. When I am not at work you can find me doing activities like snowmobiling, paddle boarding, hiking (minus the bugs!), and enjoying our rustic cabin. For the past decade, I’ve been proud to be a part of the Full Circle Health family, where I’ve grown and thrived. Now, as the Program Coordinator for the Caldwell residency, I absolutely love working with our residents and helping them grow into the doctors our small community needs.

Jessica Bartlett, FNP

Jessica Bartlett is a Family Nurse Practitioner. She completed her FNP degree at Northwest Nazarene University here locally in Nampa, Idaho. Jessica has been working for Full Circle since 2023 and is passionate about providing comprehensive primary care to patients of all backgrounds. Outside of work, you can find her traveling somewhere new, reading a good book, or hanging out with her dogs.

Dalia Chadwick is the Ryan White Medical Case Manager for Region 3 of Idaho. Working closely with the Wellness providers, nursing, reception, and pharmacy to coordinate continuity of care for patients living with HIV.

Lillian Connelly is an LCSW originally from California who has lived in six states throughout her life. She offers individual therapy and a supportive, cognitive behavioral, and skills building approach to help her clients function better and heal. Lillian is dedicated to the work she does and is open to offering consultation when it is needed, and she is available.

Seth Fankhauser graduated from the Physician Assistant Program at Touro University Nevada in 2023. He joined Full Circle Health as a Family Medicine PA, drawn by the organization’s strong commitment to affordable care and continual learning. Seth is passionate about community-based medicine and enjoys working alongside residents in a collaborative, educational environment.
Outside of clinic, Seth is an avid outdoor enthusiast and sports fan. He is excited to call the Treasure Valley home and looks forward to exploring all it has to offer.

Trudy Lizarraga was born and raised in Caldwell, Idaho, and is proud to serve her hometown as a Community Health Worker (CHW) at Full Circle Health. With a deep understanding of the community’s needs, Trudy plays a vital role in connecting patients to health education, social services, and essential resources such as housing, employment, food, and transportation.
As a CHW, Trudy advocates for individuals who may face barriers to care, helping them navigate complex systems and adopt healthier lifestyles. Her shared life experiences, cultural background, and language skills help build trust and meaningful relationships with the patients and families she supports.

Kelly Vega, MA
Nursing Supervisor
Kelly Vega serves as the Nursing Supervisor at Full Circle Health’s Caldwell clinic. She began her healthcare career in 2012 after earning her Medical Assistant certification and spent three years working as a phlebotomist before joining Full Circle Health, where she has proudly served for nearly eight years.
Born and raised in Idaho, Kelly is passionate about delivering high-quality, patient-centered care to the community she calls home. Her leadership supports both staff development and clinical excellence in a busy, team-based care environment.
Outside of work, Kelly enjoys spending time with her three children, her puppy Chiquita, and her extended family and friends. She loves music of all genres and finds joy in creating balance between work and home life.

Jessica Blancett, DO

Dr. Jessica Blancett was born in Boise, Idaho and was raised on a small farm in Meridian. She grew up working on the farm, spending time with her family in McCall tubing on the lake and snowmobiling, and working on a commercial fishing crew in Alaska during the summers.
Dr. Blancett attended undergrad at Whitworth University and while there, studied abroad in Nicaragua, Costa Rica, and Guatemala. She graduated with degrees in Biochemistry and Spanish. Following undergrad, she volunteered as an AmeriCorps VISTA for 1 year in Tucson, Arizona working at a community health center.

Dr. Blancett then attended Pacific Northwest University for medical school. She utilized her passion for providing medical care to patients in their most comfortable language by serving on the leadership teams of the Spanish Club and Global Health Club. She was privileged to return home to Idaho to complete her clinical years. Now, she is excited to be able to continue working in and serving her community as a resident at Full Circle Health.
Outside of medicine, Dr. Blancett loves to spend time with her husband, her dog, her family, and friends. She enjoys playing her violin with the worship team at her church, working on woodworking and art projects, having fun at family game nights, baking and cooking, and exploring Idaho with her husband, Nick, and dog, Cinder.

Joseph Holmstead, MD

University of Washington School of Medicine – WWAMI Program
Dr. Joseph Holmstead was raised in Middleton, Idaho where he grew up milking the family cow, showing sheep in 4H, and helping his parents and 5 siblings build what would eventually grow into Thunder Creek Llamas, renting llama pack trains to hunters and backpackers across the Northwest. Joseph is what you might call a llama whisperer. He also has plenty of experience throwing hay bales in stuffy barns and laying (lots of) irrigation pipes.
After graduating from Middleton High School, he completed two years of voluntary mission service near the cordillera of Argentina, where he fell in love with Latin America and reconnected with his mother’s Mexican heritage. He graduated from Brigham Young University with a degree in Molecular Biology while also working as a Spanish interpreter in several hospitals, jails, and schools throughout Utah Valley. He had the opportunity to work in several prominent research labs studying antibiotic resistance and novel cures during his undergraduate training. He then had the privilege of attending the University of Washington School of Medicine as a part of the Idaho WWAMI program, where he was able to complete most of his preclinical and clinical training inside his home state.
He married his sweetheart Rebecca (also from Middleton) in 2017. She is a licensed midwife and owner of Nampa Midwifery and Birth Center who is passionate about bringing gentle, evidence-informed care to the women of southwest Idaho. Joseph is dedicated to bringing an integrative, holistic approach to medicine. He is curious about the emotional and spiritual roots of disease, in addition to finding effective ways to adjust the lifestyle factors preceding many common conditions.

Joseph and Rebecca love to travel and have been all over the world, from backpacking the Icelandic highlands to celebrating a 5-day wedding in Southern India. Joseph has done extensive global health work in Honduras, Argentina, India, and Peru, and feels strongly about serving Idaho’s Hispanic population. He is an avid cyclist, placing in the top ten at Lotoja – the longest single day cycling road race in America. He completed three Ironman triathlons during medical school as well as a marathon and many cycling road races. He also loves mountain biking, backpacking, and has managed to find time to continue training llamas for his parents on weekend trips into the mountains. He is thrilled at the opportunity to complete his residency at Full Circle Health in Caldwell, just 15 minutes from his childhood home, and ecstatic to be able to serve the community that raised him.

Zach Leckenby, MD

Hello Caldwell! I was born and raised in the small rural farming community of Buhl in Southern Idaho. The middle of five brothers, I grew up living down two miles of dirt roads on the Snake River. On the property, my grandfather owned a trout farm and a hydroelectric power plant. My father worked for the local fertilizer company while my mother worked for the Buhl School District driving a school bus. With my brothers I spent most of my time outdoors catching snakes and lizards, and fishing along the river. I also developed a strong interest in sports at an early age and played football, basketball and baseball for the Buhl Indians.

I went on to become the first person in my family to graduate from college when I earned my undergraduate degree from Boise State University. With a desire to stay in Idaho and improve healthcare in the state, I then attended the University of Washington School of Medicine. During medical school I participated in the Targeted Rural & Underserved Track (TRUST) where I gained additional experience in rural primary care at the St. Lukes family medicine clinic in Jerome, ID. My career interests include rural primary care here in Idaho for our farming communities and for our Idaho veterans. I wish to develop long-lasting relationships with my patients and to become an integral part of my local community.
My interests and hobbies include history, notably WWII aviation, sports and sturgeon fishing. I also love food, and some of my favorites include chicken fried steak and Mexican food. I am proud to call Idaho home and am excited to begin my career as a physician for my fellow Idahoans.

Savannah Whiting, DO

Savannah was raised in Idaho Falls, Idaho as the youngest of 3 children. She attended school and graduated from Idaho Falls High School in 2013. She subsequently pursued her college education at both Treasure Valley Community College and Peru State College. Here, she earned a bachelor’s degree in Disease and Human Health while also competing on her university’s collegiate volleyball team for 4 years.

After graduating from undergrad, Savannah began working as a phlebotomist at Saint Alphonsus Regional Medical Center in Boise. Here, her love for clinical medicine and working with patients cemented her desire to attend medical school. In 2020, she was accepted to the Idaho College of Osteopathic Medicine and began her journey to becoming a physician. She is excited to join the Full Circle Health Family Medicine Residency in Caldwell.
Savannah has several interests within family medicine but a few that she is most passionate about include women’s health, rural medicine, endoscopy, and sports medicine. In her spare time, she enjoys spending time outdoors, playing volleyball, attending CrossFit classes, and playing with her dog, Ken. Kayla was born and raised in Spokane, Washington with her older brother and fraternal twin sister, aside from a few years when they lived in the Caribbean. After graduating high school in 2017, she attended the University of Idaho where she earned a bachelor’s degree in psychology and minors in Spanish and Pre-Health Professions. U of I is also where she was lucky enough to meet her now husband, who was born and raised in the Treasure Valley. Kayla also spent much of her childhood in Idaho because most of her relatives live throughout the state, and she has many wonderful memories with them picking huckleberries, riding horses, and helping on her grandfather’s farm.

Kayla has always aspired to become a physician and after graduating from college, she attended the Idaho College of Osteopathic Medicine. This is where her love for preventative medicine grew, and she felt called to become a family physician and serve our community. She is very excited to continue living in the Treasure Valley and join the Full Circle Health Family Medicine Residency in Caldwell!
Kayla has many interests within family medicine but a few that she is most passionate about include women’s health, mental health, rural medicine, and lifestyle medicine. In her spare time, she enjoys exploring the outdoors, traveling, reading, and trying new foods. She has been to 30 countries so far and can’t wait to explore all seven continents with her loved ones.

Hailey Graviet, MD

Dr. Hailey Graviet was raised in Nampa, Idaho and graduated from Skyview High School. She attended college at Brigham Young University in Provo, Utah, majoring in physiology and developmental biology. Prior to graduating, she spent 18 months serving a voluntary mission in Vietnam where she became fluent in Vietnamese. Upon returning to BYU, she met and married her husband, Ian. Hailey attended medical school at the University of Utah and led multiple community outreach programs focusing on promoting health and wellness in pediatric and Asian American populations. She was able to complete some of her clinical rotations in Boise, which fostered her desire to return to Idaho and serve her local communities.
Hailey’s professional interests include preventative medicine, women’s health, pediatrics, and dermatology. Outside of medicine, Hailey loves baking, listening to podcasts, taking long walks in good weather, retail therapy, and her new baby boy who was born in December of 2024.

Caleb McKee, DO

Dr. Caleb McKee was raised in Twin Falls, Idaho, where he developed a love for the outdoors—rock climbing, kayaking, and mountain biking around the Snake River. In high school, he enjoyed playing soccer and performing in the show choir Synergy. Before college, he served a two-year voluntary mission in Slovenia, Croatia, and Serbia, which deepened his passion for service and global health.
He earned his bachelor’s degree in biomedical sciences at BYU-Idaho, where he met his wonderful wife, Morgan. Before medical school, he worked as a program director for a medical scribe program in Washington, overseeing five primary care clinics. He later became part of the inaugural class at Noorda College of Osteopathic Medicine in Provo, Utah. During his education, he and his wife welcomed four children—two during undergrad and two more during medical school- Making the journey even more rewarding.

Dr. McKee is passionate about providing holistic care, integrating mind, body, and spirit into his diagnostic and treatment approach. He enjoys incorporating osteopathic manipulative treatment (OMT) into his care. He has a special interest in preventative medicine and global health having participated in medical humanitarian work in Fiji—and he hopes to continue similar efforts in the future. He is very excited to be a part of Full Circle Health’s Family Medicine Residency and looks forward to honing his skills and serving this community.
Outside of medicine, Dr. McKee enjoys biking, hiking, playing soccer, and video games with his brothers. He and his wife are avid board gamers, with a collection of over 100 games that they enjoy playing with friends and family. Above all, he enjoys spending time with his wife and children, making memories in everything they do together.

Dylan Quintana, MD

I grew up in the suburbs of Boise, Idaho. Originally born in Orlando, Florida with my twin brother Apollo. My family moved to Idaho to be closer to my grandparents when I was 3 years old. I spent my childhood playing soccer, reading, and racing mountain bikes with the BYRDS cycling team. From a young age, I was passionate about science, anatomy, and helping people. I knew I wanted to be a physician and take care of my community.
After graduating from Rocky Mountain High School, I attended the College of Idaho in Caldwell and earned a Bachelor of Science in Chemistry. I also met my wife, Isabela, who has been stuck with me for 10 years. In between giving me frequent reminders of how lucky I was to meet her; my wife became a physician assistant. During college and after, I worked as a certified nursing assistant and emergency department physician scribe for St. Luke’s Health System. These clinical experiences expanded my knowledge of medicine and gave me foundational lessons in how to take care of people. I went on to attend the University of Washington School of Medicine as part of the Idaho WWAMI class.

I am extremely excited to be returning to the Caldwell community, and Idaho in general, so I can live and train in the area I have long called home. My interests in medicine are broad but I have specific interests in sports medicine, preventative health, and lifestyle interventions. My past experiences as a CNA and ED scribe have shown me that quality of life is just as important as length of life; my goal is to allow my patients to live their lives to the fullest through a collaborative doctor-patient relationship, evidence-based medicine, and holistic solutions to their health concerns.
Outside of medicine, I love spending time with my wife and two doodle dogs. My hobbies and interests include weightlifting, mountain biking, reading, video games, eating great food, and fantasy football. I am beyond thrilled to be able to complete my residency training with the fantastic Full Circle Health program in Caldwell.

Alec Akins, DO

Dr. Alec Akins is originally from Hugo, Minnesota, a small town north of the Twin Cities. He earned his undergraduate degree in Biology from Iowa State University in Ames, Iowa, and went on to attend the Idaho College of Osteopathic Medicine. Alec grew up playing multiple sports, with a particular passion for hockey and football, which sparked his interest in musculoskeletal health, performance, and injury prevention. He continues to stay active through golf, strength training, and outdoor activities.

Alec chose Family Medicine for its broad scope and its ability to provide comprehensive, hands-on care across diverse patient populations. Having grown up working on a farm, he developed a deep appreciation for rural life and is passionate about serving rural communities, particularly farmers, laborers, and athletes. His professional interests include full-spectrum rural medicine, procedural care, and sports medicine, with a goal of helping patients optimize both health and performance. He plans to pursue sports medicine fellowship and remain in Idaho long-term.
Alec and his wife, Madi, have known each other since childhood and began dating at age 13 before getting married in Idaho in 2023. They have a daughter, Lorelei, and two Australian Shepherds, Finley and Dorothy. In his free time, Alec enjoys spending time with his family, hiking, exercising, and taking advantage of Idaho’s outdoor lifestyle. He also likes to volunteer at Domini farm Animal Sanctuary in Kuna, Idaho.

Joshua Allred, DO

Dr. Joshua Allred grew up in the rural town of Raymond, Alberta, Canada. He is the 7th of 9 children and had a fun-filled childhood, including singing and dancing in the Raymond High School Show Choir. After serving a church mission in Winnipeg, Canada, he enjoyed working with his older brother in landscaping. He then attended BYU-Idaho where he met his beautiful wife, Stephanie, who is from Cottonwood Heights, Utah. They were married in 2018 and have 2 wonderful children: Matthew and Ellie.
Throughout his education, Joshua has volunteered in various community service projects, including Remote Area Medical and Global Medical Brigades, reaching rural and underserved populations. He also greatly enjoyed his time in the BYUI Neuroscience Academic Society, providing outreach to high school students. At BYU-Idaho, studying anatomy and physiology drew him to medicine, but volunteering in the community and working as a caregiver and phlebotomist are where he came to love serving people, which solidified his desire to be a physician.

During medical school at Pacific Northwest University, Joshua welcomed the opportunity to do clinical rotations all around the Treasure Valley, which is when he became passionate about family medicine. Creating meaningful patient-doctoral relationships where all feels heard and involving patients in shared decision making are important elements of Dr. Allred’s care. He desires to serve people of all ages in family medicine and is excited for the next stage of his career.
In his spare time, Joshua enjoys basketball, golf, singing, reading, playing board games, and especially spending time with his family. He and his wife love Idaho and the people of the Treasure Valley. Joshua is excited to continue his education right here in Idaho at the amazing family medicine program of Full Circle Health – Caldwell!

Isaac “Ike” Burton, DO

Dr. Isaac Burton was born and raised in Ogden, Utah, with a short but memorable stint in Moscow, Idaho. As the third of nine kids—eight of them boys—he grew up in a lively household that taught him teamwork, resilience, and how to hold his own. He spent his younger years wrestling, racing mountain bikes, and running cross-country, and later worked four summers at a scout camp near Ashton, Idaho, where he led hiking trips through the Tetons and Yellowstone.

After high school, Isaac traded the mountains for the city and served a volunteer mission in Buenos Aires, Argentina, where he fell in love with the Spanish language and culture. He went on to study Spanish at Utah State University before heading to Des Moines University for medical school.
Now, he’s thrilled to be returning to Idaho for residency—back to the mountains, fresh air, and the kind of community he’s always felt at home in. He is passionate about full-spectrum family medicine, with a particular interest in sports medicine, and especially enjoys building long-term relationships with patients and caring for entire families across generations.
Isaac is married to his high school sweetheart, and together they have three energetic kids who keep life loud, busy, and fun. When he’s not in the clinic, you’ll likely find him fly fishing, mountain biking, or humbly trying (and often failing) to improve his golf game.

Jackson Sterling, DO

Dr. Jackson Sterling grew up in Northern California in the town of Lincoln. He attended the University of Nevada, Reno where he studied biology. Before medical school, he spent his undergraduate years working a variety of jobs including roofing, driving forklifts, and working as a respiratory aide. He went on to earn his medical degree from the Idaho College of Osteopathic Medicine.
He was drawn to family medicine because of its broad scope, the opportunity to build long-term relationships with patients, and the ability to care for individuals across all stages of life. His clinical interests include preventative medicine, obstetrics, procedures, and rural medicine.

Jackson’s connection to Idaho began in high school when he developed a passion for fly fishing. What started as hours spent reading about rivers and studying maps eventually led him to discover Idaho. Since then, he has hoped to make the move to Idaho and was excited when he was accepted to attend medical school there. Since moving to Nampa, he has come to appreciate Idaho’s communities, landscapes, and opportunities for practicing rural medicine. He married his wife Jessica in 2021. Jessica is a registered nurse who also shares a passion for caring for others, and together they look forward to continuing his training in Caldwell and serving the people of Idaho.
Outside of medicine, Jackson enjoys spending time outdoors, especially fly fishing, as well as camping, cooking, gardening, and playing with his blue heeler, Carson.
