Full Circle Health – Caldwell Clinic

Full Circle Health (FCH) is happy to provide family medicine and behavioral health care services at their new clinic space on the 2nd floor of the Elm Street Clinic in Caldwell!

The following providers will be transitioning to Full Circle Health:

If you have Medicaid as your health insurance provider and would like to continue seeing one of the above named providers for your care needs, you will need to contact the Idaho Department of Health and Welfare, or ‘Healthy Connections‘ to update your Primary Care Provider (PCP) to FULL CIRCLE HEALTH.

*NOTE* Patients can select “Full Circle Health – Nampa South Clinic” or “Full Circle Health – Nampa North Clinic” when updating their Primary Care Provider (PCP). The Caldwell Clinic opens July 5th, so as it may not be an option to select prior to July 5th, you will still be able to be seen in Caldwell by choosing one of the Nampa locations. “Enrollment in Healthy Connections will be with a clinic, not a specific provider. The clinic will work with you to determine your specific primary care provider or primary care team.” Visit “Manage my Medicaid Page” for more details.

Patients who are on Medicaid can only select/change their Primary Care Provider (PCP) once per year, during open enrollment starting July 1, 2023 (ending July 31, 2023). It is required that Medicaid patients choose their PCP for billing purposes, so this step is important if you would like to continue seeing one of the above named providers.

Full Circle Health will start seeing patients on Wednesday, July 5th!

Who or what is Full Circle Health?

Full Circle Health is a non-profit, federally qualified health center (FQHC) with a Medical Residency which is commonly known as a Teaching Health Center (THC).  Full Circle Health provides a wide range of medical and behavioral care services to approximately 30,000 patients across the Treasure Valley.

What is a Teaching Health Center?

Teaching Health Centers are the solution to the troubles facing primary care in the United States. Across the nation, fewer and fewer medical school graduates are pursuing careers in primary care – just as many working primary care providers are planning to retire. Teaching Health Centers aim to ensure a capable primary care workforce for low-income communities by providing training at federally qualified health centers and similar sites. Research shows that residents who train in federally qualified health centers (FQHCs) are more likely to practice in medically underserved areas – and to feel more prepared to do so. A resident doctor is already a graduate of a medical school, licensed to practice, and trained as part of the graduate medical education (GME) program to take care of patients within the clinic and nearby hospitals.
