Insurance Details:
We accept most major insurance plans, including Medicaid (Idaho & Oregon) and Medicare.
If you are uninsured we offer a Sliding Fee Scale:
Services, including specialty care services, are covered for sliding fee patients. Full Circle Health offers a sliding fee discount program to eligible persons based on the patient’s ability to pay. Ability to pay is determined by the household size and annual income relative to a discount schedule based on federal poverty income guidelines.

Free Short-Term Counseling Option:
If you would like to be seen by a behavioral health intern in-training, we offer short-term counseling with these trainees completely free of charge. Please ask us for this option at the time of scheduling your appointment.
Out-of Pocket Price Transparency:
Individual Counseling: 45-50 minutes, $200.
Couples Counseling: 45-50 minutes, $200

Kevin Caskey, LCSW
Chief Behavioral Officer
Kevin Caskey, LCSW is a graduate of the University of Tennessee School of Social Work. His professional experience includes 7 years in FQHC settings. Prior to this, he worked in residential, intensive outpatient and private practice settings. In terms of practice scope, he works with adults (25+). Treatment areas include depression, anxiety, trauma and addiction/substance abuse. He also has expertise in the area of men’s mental health.

Lillian Connelly, LCSW
Behavioral Health Clinical Team Supervisor
Lillian is from Northern California, although she has resided in five states in her adult life. She is a U.S. Air Force veteran who, through lived experiences, became acquainted with and began to pursue a career in social work. She graduated with her master’s in social work in South Carolina in 2017 and has lived in Idaho ever since. Lillian has experience working in community based mental health clinics with various populations, to include children, teens, young and older adults. She offers an indirect, person-centered approach along with guidance in the areas of mindfulness based, motivational interviewing, CBT, and a problem solving or solution focused approach. Lillian is passionate about the dignity and worth of each person she works with and strives to help people live their best lives. In her free time, Lillian enjoys spending time with her family. She enjoys reading, writing, crochet, and outreach activities.

Adam Burwell, LCPC
Adam was born in Boise and grew up in Meridian. He attended the University of Idaho where he earned a bachelor’s degree in education. Toward the end of his 12 years of teaching elementary school in Meridian, he was also completing a master’s degree in professional counseling from Grand Canyon University. He became a licensed counselor in Idaho in 2022 and practiced in a small agency setting prior to joining Full Circle Health. He practices using a person-centered approach and incorporating Motivational Interviewing and Cognitive Behavioral Therapy interventions to address adjustment, anxiety and mood disorders. He works out of the Nampa South clinic and is particularly passionate about helping teens and young adults enter and navigate the mental and emotional health world.

Caroline Caudill, LMSW
Caroline Caudill is a Licensed Master Social Worker (LMSW) at Full Circle Health. She earned her master’s in social work from Boise State University and completed her internship at the Boise VA, gaining extensive experience working with veterans experiencing Substance Use Disorders, Sexual Trauma, and PTSD. Caroline is also a certified Cognitive Processing Therapy (CPT) provider.
Before entering social work, Caroline was a Special Education teacher, working closely with individuals diagnosed with ASD, ADHD, and Dyslexia. This experience strengthened her expertise in supporting families and fostering healthy interpersonal relationships.
Originally from Florida, Caroline is married with three children. Her family has a strong sailing background, often spending months at a time on the water, and they are now adjusting to land-based activities. When she’s not working, you’ll likely find Caroline walking the Greenbelt with her dogs and family, often stopping for a coffee and a snack. Caroline integrates evidence-based modalities such as Acceptance and Commitment Therapy (ACT) with a value-driven focus, Dialectical Behavior Therapy (DBT), Cognitive Processing Therapy (CPT), and person-centered therapy, while also drawing from strength-based and mindfulness approaches. She incorporates a somatic and trauma-informed perspective, helping clients build safety, body awareness, and resilience. With compassion, empathy, and understanding, Caroline supports clients in exploring their experiences at a pace that honors both their mind and body.

Hannah Max, LPC
No two journeys to seeking therapy are the same. As a Licensed Professional Counselor, I am passionate about bearing witness to each person’s unique story. Whether you are in the first, frightening steps of making a big life change, or are searching for connection and empathy as you face difficulties, whether you have been receiving therapy for many years or are giving it a try for the first time, I see it as my sacred privilege to walk your path alongside you.
My approach to providing mental health services is based on the fundamental belief that humans need connection: to others, themselves, the world around them, and to a sense of purpose. I use evidence-based best practices to meet each client where they are and foster the connections they most desire, in order to empower each client to live their fullest life. You can expect a collaborative process in session with me, which could include mindfulness techniques, solution-focused interventions, and will always be based on active, empathetic listening.
I am a firm believer that the counseling services I provide should be dynamic and flexible, in order to provide the best possible care for each individual. I prioritize cultural humility, validating client experiences, and building relationships with clients. My primary clinical interests include women’s issues, LGBTQIA+ issues, life transitions, and developing healthy intra- and interpersonal relationships. When I’m not working with clients, you can find me reading a new book, cooking for my loved ones, or anywhere with a view of the sun or the stars.

Jeffery Meador, LAMFT
Jeffery Meador’s path to becoming a therapist was shaped by personal growth, education, and a strong commitment to helping children and families. After starting his studies at Union College, Jeffery initially pursued International Rescue and Relief but later changed his major to social work, earning his bachelor’s degree in the field. After college, he worked at Sunset Lake Camp and later at Camp Blue Ridge, where he grew as a leader and mentor. Following those experiences, Jeffery returned to Nebraska to work as an IFP skill builder, providing direct support to teens and families as they developed life skills. Driven by a desire to expand his knowledge, Jeffery earned his Master’s degree in Marriage and Family Therapy at Loma Linda University.
After completing his master’s degree, Jeffery joined Project PATCH in Garden Valley, Idaho, where he worked with teens in a residential treatment setting, helping them navigate complex challenges and build a foundation for healing. Today, Jeffery works at Full Circle Health, supporting children and families with a focus on compassion, collaboration, and growth. His approach is grounded in empathy, patience, and a belief in every family’s ability to thrive.

Mazie Scott, LCPC
Mazie holds a bachelor’s degree in psychology from the University of Idaho (2018), a master’s degree in clinical mental health counseling from Corban University (2020) and is a Licensed Clinical Professional Counselor in Idaho and a Licensed Professional Counselor in Oregon since 2024. Mazie has clinical experiences from working in university mental health care, psychiatric crisis / community care, intensive in-home youth and family counseling, and pediatrics healthcare. Mazie primarily works with adults at Full Circle Health, however, does support limited adolescents and teens, with primary concerns of anxiety, mood disorders, life transitions, Christian faith integration, stress management, behavioral problems, and grief and loss counseling. She practices using Cognitive Behavioral Therapy, Acceptance and Commitment Therapy, Mindfulness and Self-Compassion, Person Centered approaches, and Solution Focused approaches. She is passionate about supporting the lifespan of individuals who keep facing forward no matter what they are faced with. In her free time, Mazie enjoys quality time and traveling with her husband and newborn daughter.

Winslow Gerrish, PsyD
Director of Behavioral Sciences, Research and Grants
Dr. Winslow Gerrish received his Ph.D. in clinical psychology from Seattle Pacific University and completed an internship at the VA medical center in Portland, Oregon. Before coming to Full Circle Health, he acted as director of the PTSD clinic at the Boise VAMC and served as faculty in their Center of Excellence in Primary Care Education. Dr. Gerrish grew up the son of a rural, full-spectrum, family doc in a small dairy town in Washington State. This upbringing laid the foundation of his life-long passion for family medicine and caring for the underserved.
Dr. Gerrish is a Clinical Assistant Professor of the University of Washington and has been involved in primary care education with multiple University of Washington and WWAMI residency programs in Boise and Seattle. His special interests include PCMH transformation, interprofessional education, and behavioral health integration in primary care. His recent publications include a new book, Interprofessional Education in the Patient-Centered Medical Home. Dr. Gerrish is an avid climber and skier. When not at work, you will find him with his wife (also a family physician) and children trying to get lost in the mountains while engaged in some type of crazy outdoor adventure.

India King, PsyD
Director of Behavioral Health
India received her doctorate in Clinical Psychology with a focus in Health Psychology from Pacific University in 2014. She completed her doctoral internship at the White River Junction VA in Vermont and her postdoctoral fellowship at the Boise VA, with a focus on integrated primary care at the Center of Excellence in Primary Care Education (CoEPCE). Dr. King’s passions are graduate medical education and interprofessional education and have been a core component of many of her roles. Dr. King’s clinical and research interests include primary care psychology, chronic disease management, interprofessional teaching, shared-decision making and motivational interviewing. You can find Dr. King primarily at the Nampa Clinic for services! When Dr. King is not hard at work serving Full Circle Health patients and our respective community and involved in teaching residents, you can find her enjoying the outdoors on her mountain bike!

Ahva Mozafari, PhD
Psychology Training Director
Dr. Ahva Mozafari grew up in Boise, ID and earned a bachelor’s degree from Boise State University and a master’s degree from Eastern Washington University. She practiced as a Mental Health Therapist in Boise and Nampa before pursuing a Ph.D. in Clinical Psychology at Idaho State University. She completed her clinical internship at the Michael DeBakey VA Medical Center in Houston, TX and then joined the faculty at Idaho State University where she served as the director of the psychology clinic. She joined Full Circle Health in 2024 as the Psychology Training Director. She has special interests in psychology training, health psychology, women’s health, and trauma/anxiety treatment.
